Why dental scaling twice a year is essential

The explanation from our dental hygienist at Centre Dentaire du Plateau.

By Dr. Saul Barchichat

It is one of the most common questions we hear at the clinic: "I haven't been to the dentist in a couple of years — is that a problem?" Often, the answer reveals that the patient has not been seen for 2 or 3 years. Dental prevention — particularly professional scaling — is the best way to keep your mouth healthy and avoid costly treatments.

What is tartar and why is it problematic?

Dental plaque is a bacterial film that forms naturally on your teeth after every meal. If not removed through regular brushing and flossing, it mineralizes within 24 to 72 hours to form tartar (dental calculus). Once calcified, tartar can no longer be removed by brushing — it requires professional scaling.

  • Tartar irritates the gums and causes gingivitis
  • Left untreated, it progresses to periodontitis (severe gum disease)
  • Periodontitis is the leading cause of tooth loss in adults
  • Links have been established between periodontal disease and heart disease

What is a professional dental cleaning?

During a scaling (or dental prophylaxis), our hygienist uses ultrasonic and manual instruments to remove supragingival (above the gum) and subgingival (below the gum) tartar. The treatment also includes polishing tooth surfaces and personalized hygiene advice. The appointment lasts approximately 45 to 60 minutes.

  • Ultrasonic scaling (comfortable and effective)
  • Subgingival scaling for deep areas
  • Polishing to remove surface stains
  • Personalized brushing and flossing advice

Why twice a year and not once?

The recommendation for two visits per year is based on the time it takes tartar to accumulate to a clinically significant level — generally 6 months. Some patients (smokers, diabetics, people with a history of periodontitis) may need up to 4 cleanings per year. Conversely, patients with perfect hygiene and little tartar may only need to visit once a year. Your hygienist will adjust the frequency to your situation.

What prevention actually saves you

A professional cleaning twice a year typically costs $100 to $200 per visit. By comparison, treating advanced periodontitis (root planing) can cost $800 to $2,000, not counting the extractions or implants that may follow. Prevention is clearly the most cost-effective investment for your dental health.

  • Prevents gingivitis and periodontitis
  • Detects cavities at an early stage (less treatment needed)
  • Reduces the risk of long-term tooth loss
  • Far less expensive than curative care

Frequently asked questions

Does scaling hurt?

For the majority of patients, scaling is not painful. Some sensitivity is possible if the gums are inflamed or if there is heavy tartar buildup. We use ultrasonic instruments that minimize discomfort, and we can apply a numbing gel if needed.

Is my cleaning covered by insurance?

The vast majority of dental insurance plans cover 2 professional cleanings per year at 80-100%. The CDCP also covers preventive hygiene care for eligible patients.

Do I need a cleaning even if my teeth don't hurt?

Absolutely. Most serious dental problems (deep cavities, periodontitis) are asymptomatic in their early stages. Waiting until something hurts usually means waiting until the problem is much more severe — and much more expensive to treat.

Does an electric toothbrush replace professional scaling?

No. An electric toothbrush is excellent for removing plaque daily, but it cannot remove calcified tartar. Only a professional with appropriate instruments can do that.
